FORT WORTH - Mike Sivek, 56, a paint contractor, died Sunday, Jan. 18, 2004, in Fort Worth. Funeral: 9:30 a.m. Wednesday at Mount Olivet Chapel. Burial: Mount Olivet Cemetery. Visitation: 6 to 8 p.m. Tuesday at Mount Olivet Funeral Home. Mike was born July 26, 1947, in Fort Worth. He was preceded in death by his father. Paul Sivek, in 1999.
